We have an opportunity for you to join our ground breaking Reablement service as an Occupational Therapy Assistant.
Our vision is to support people in the community to live the best lives they can, and our Reablement Service is part of this fantastic new approach to influence people’s lives in a person-centered and positive way.
**LIVE **WORK** SUPPORT**
Reablement focuses on what people can do and builds on these skills rather than focusing on what the individual cannot do. We work with community partners and organisations to re-connect the individual to activities and groups that have been or are important to that person.
This improves well-being and reduces loneliness. The Reablement team seeks to support people to make use of community and personal assets to achieve their goals.
**About the role**
You will work with people in their own homes and play a key role in enabling customers to achieve their goals and objectives. Working alongside dedicated Occupational Therapists, you will co-deliver our customers’ reablement plans.
In addition, our new service is moving towards an integrated relationship with our health partners, and we have designed a new discharge pathway that supports customers on their journey home from hospital in a seamless whole system approach called Home First Plus. This will see health and social care staff working together to achieve the best outcomes for the customer.
