Community Mental Health Adults Locality Pathway Allocation Co-ordinator Band 7
This is a unique opportunity to be one of our Locality Pathway Allocations Co-ordinators for Community Mental Health covering the South Warwickshire Area.
This role is unique and involves co-ordination and oversight for requests for mental health assessment and intervention for adults. This role not only involves working with health and social care colleagues, but also the wider services such as voluntary sector and other statutory services who may need your advice and support and form a key part of our pathway allocation.
Your base can either be in Leamington Spa or Stratford upon Avon, but flaxibility is needed to travel between sites.
You will oversee the requests coming into the locality and ensure that the patients are put on the right pathway and seen by the right person at the right time.
You will work closely with colleagues in community mental health and urgent care services to support these pathways.
The transformation of community mental health services means that we have an impending target of four weeks from referral to treatment. This role is vital in ensuring that we are meeting this target and that we are monitoring our outcomes closely.
The transformation is linked to a number of new and exciting developments that will directly and indirectly link to this role as we have a number of new roles and services with the Trust.
You will be a key part of bringing together different organisations and therefore excellent communication and IT skills are essential.
You will be managed by the Operational Place Manager and be part of the wider community mental health management team. The management team meet on a regular basis and support the pathway allocation process.
You will work alongside our administrative colleagues to ensure a smooth process for those we serve.

You will lead the regular Locality Pathway Allocation Meetings that involve a range of services, clinical and administrative staff.
You will ensure that requests for intervention or advice are dealt with promptly and allocated to the correct pathway according to their presenting needs. This must be communicated with patients and referrers alike promptly and with clarity.
You will work alongside the wider management team and ensure that any bottlenecks or changes in demand are communicated effectively to problem solve and prevent any undue delays.
Excellent organisational skills are essential for this exciting and challenging role.
