Health Liaison and Diversion Facilitator

An exciting opportunity has arisen to recruit a band 4 Health, Liaison and Diversion Facilitator (HDLF).

The HLDF will have the responsibility for providing quality health care to individuals in contact with criminal justice agencies. They will offer and provide a comprehensive screening to individuals to identify physical health concerns, mental health concerns and other vulnerabilities.

The post holder will also require additional enhanced Police vetting checks which can take an additional 12 weeks to obtain.

Working within police custody, court and community providing support with practical concerns or difficulties that might be affecting mental health and wellbeing; supporting individuals to access the professionally qualified team members when clinically appropriate. The post holder will develop, enhance and improve their knowledge of the interface between criminal justice and health and social care agencies, developing skills and competencies at all opportunities.

The position is based across ALL Cheshire Custody Suites and will operate from 7am to 10pm, 7 days a week.

**The HLDF will**: Provide high quality information to key decision makers in youth and criminal justice agencies, including the police, courts, probation and Youth Offending Teams (YOTs) Secure referrals into mainstream health and social care services.
Provide brief evidence based intervention to promote health and wellbeing
Actively seek to embed service user consultation and involvement in
every aspect of service provision.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.
