We have an exciting opportunity in our Cornwall services for two Hospital Outreach Workers to join our busy and dynamic team in Truro.
This is an exciting and challenging role that works from a trauma informed perspective.
The Hospital Outreach Service provides a rapid response to individuals who are frequent attenders at the Royal Cornwall Hospital in Truro and have issues related to substance use.
The team works within a multi-disciplinary model of practice and partners with safeguarding, housing and homelessness services, domestic abuse services, as well as primary and secondary health care and mental health providers.

**Essential Skills** We are looking for someone with outstanding communication skills and the ability to work effectively as a team player in a collaborative and supportive manner.
We need someone who thrives under pressure, has good time management and prioritisation skills, and can work well with others.
In addition, we are looking for someone who is positive, confident, and has previous experience of working with substance misuse service users in a health, social care, or criminal justice setting.
The team provides a countywide response so you will require a current clean driving licence and access to a vehicle for this Hospital Outreach Worker role.
