Following some internal moves, we are looking for a two Home from Hospital Facilitators to join our team.

You will be identifying the patients requiring additional support and ensuring a safe and timely transition back following a stay in hospital.

This could include practical tasks such as shopping or topping up key meters alongside promoting the customer’s independence by providing support on tasks such as benefits, debt management and budgeting.

Service outcomes will improve hospital patient flow, avoid bed blocking, tenancy sustainment, reduce social isolation, access to benefits and support access to training, education and employment.

**About you**:
You will have experience working in a support or care environment, providing excellent customer service. You will also have genuine empathy with the purpose of housing association and support services.

As you will be supporting a range of customers, with different needs, excellent communication skills, both written and interpersonal are needed.

You will also be flexible, meeting the needs to the customer by occasionally working outside of your traditional office and hours.

Due to the nature of this role, you will need to be able to drive with access to your own vehicle.
