Working collaboratively with our partners across Primary Care, Diabetes UK and Public Health, we are dedicated to supportingindividuals to improve their health to their risk of type 2 diabetes.

We are now looking for enthusiastic and supportive Health Coaches in the Black Country Area. Health Coaches form a key part of the Diabetes Prevention Programme Team at Ingeus. Fundamental to this role is experience of Coaching/Mentoring and an ability tosupport participants achieve improved health outcomes. Applicants are not required to be health care professionals, however delivery of preventative health, public health or health awareness is essential, as is an interest in diabetes.

**The role is mainly face to face delivery of course content (dependent on guidelines) to groups of 25 therefore driving license and access to a car is essential. Due to the nature of the delivery some evening and weekend work may be necessary.**

**Role Responsibilities**
- Delivering the NDPP curriculum to participants, ensuring participants achieve and experience the right learning outcomes.
- Responsible for the accurate entry of participant data and management of a participant caseload.
- Managing participant feedback & experience of the National Diabetes Prevention Programme (NDPP)
- Managing programme performance & data management systems
- Work effectively with the NDPP Programme schedulers & managers to ensure the successful delivery of the NDPP to expected outcomes.
- Carry out self-evaluation after delivery of every NDPP education courses
- Participate in the NDPP Training & Quality Development Framework
- Offer support to the curriculum development team regarding appropriate cultural tailoring of programme content.

**Requirements**:

- Applicants are welcomed with multi language skills
- Completion of full training delivered upon induction by Ingeus
- Applicants will be required to study in own time (using self development work books, curriculum, training materials & session preparation)
