The Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent has been designed to the highest level of specification. This modern purpose designed unit is the central point for all those in need of a Crisis Assessment/Intervention. Teams within the Crisis Care Centre include the All Age Access Team, Home Treatment Team, High Volume Users Team and a commissioned place of safety for those detained on a section 136.
This role is integral to the Trust’s full adoption of Quality Improvement (QI) model. The post holder is responsible for supporting the planning and oversight of the Quality Improvement programme which will oversee all QI activity, alongside facilitating the delivery of Quality Improvement training and oversight, monitoring and reporting on Trust wide QI projects.
The QI Facilitator will report to the QI Lead Practitioner and work with Directorate Quality Improvement Leads, providing advice, coaching support and by drawing on their own skills, practice and experience will help to influence the direction for Directorate QI work by supporting the analysis of a wide range of quality metrics including performance data and feedback.

A visible leader for Quality Improvement methodology and able to use motivational skills in order to work collaboratively with Trust Leads and clinical teams undertaking QI activity:
Supporting development and deployment of Trust QI strategy
- Ensure that Quality Improvement is being delivered in line with the Trust’s strategic vision
- Communicate with staff at all levels about the importance and relevance of Quality Improvement
- Contribute to the Quality Improvement communication plan which includes social media
- Work with teams to ensure all Quality Improvement projects are registered and managed on LifeQI system
- Attend regular Senior Management meetings, providing support and information as required; ensuring action points are followed through to ensure completion.
- Present progress of Quality Improvement projects to both internal and external clinicians and senior managers at a range of meetings and forums.
- Work with teams to ensure Quality Improvement projects are displayed and success celebrated
- Provide a link role between the Quality Improvement Lead Practitioner and Clinical Directorates to support the development of their QI portfolios
2.2. Leading QI on the frontline
- Work alongside clinical and non-clinical staff at all levels to promote and support Quality Improvement projects that support the delivery of the Trust’s objectives
- Use Quality Improvement techniques to describe and analyse current processes pathways from a clinical and administrative perspective, identifying recommendations for pathway changes to ensure an effective system is in operation
- Show leadership of Quality Improvement through coaching/supporting teams to design & carry out tests of change via Plan-Do-Study-Act (PDSA) cycles
- Work with teams to identify process, outcome, and balancing measures for improvement projects and develop data collection plans
- Use data to analyse change to identify improvements and celebrate success
- Support the Quality Improvement data analyst function, using data for improvement to understand processes and diagnose problems to identify improvement opportunities and where efforts should be focused
- Assist both internal and external clinicians and Service Managers in the development of care pathways that may span across more than one organisation
- Use Quality Improvement techniques to describe current processes pathways from a clinical and administrative perspective, identifying recommendations for pathway changes to ensure an effective system is in operation, supporting change management programmes as appropriate
