**Contract**:30 hrs/wk over 4 or 5 days Mon - Fri (negotiable) - some weekend work may be required on occasion **Salary**: £22,680.90 (Pro rata £18,144.72) **Location**:Champions House, Wrexham **Project overview**: The new Prison In Reach service in North Wales is designed to give an opportunity for people seeking recovery, with a history of substance use, and that are patients of the substance misuse service within the prison estate to be referred for support both pre and post release. Engagement with the service will be voluntary and not associated with any mandatory requirements. It will dovetail into the existing collaborative outreach model of service which enable early intervention and access into critical services to ensure a smooth transition in to the community. Services such as substance misuse services, mental health services, housing services, volunteering, employment and training services will be key delivery partners. This will all be provided in a supportive and person centred way. **Role purpose**: The post holder will be required to co-ordinate the delivery of recovery support for people leaving prison. The coordinator will work closely with Adferiad and Kaleidoscope and other partners to secure a smoother transition into community from custody. The coordinator will coordinate/provide assessment, recommendation, and feedback to colleagues within Dechrau Newydd/Adferiad and Substance Misuse Services on the suitability and recovery progress of substance misuse service users within the Criminal Justice system. The post holder should enjoy face-to-face contact with people who have recognised that their use of substances has become problematic and has direct affiliation to criminality. They will have excellent communication skills, be able to work closely with other multi-disciplinary agencies and be able to work flexibly and efficiently to ensure services are delivered equitably and at times to suit the service user. Furthermore, the post holder will be confident in networking, building relationships and capital and leading a Team of ‘Volunteer Community Connectors’. The post holder will be expected to work in line with the Social Services and Well Being (Wales Act) 2014, and the Violence Against Women Domestic Abuse and Sexual Violence (Wales) Act 2015 to ensure safeguarding of individuals (adult and children) is at the heart of service delivery. A good understanding of confidentiality and information sharing legislation is required with the confidence to act professionally and seek guidance and support from line management and colleagues where appropriate. **Responsibilities**: **Administration** - To keep up to date records of service user work (case notes) data entry and reports as and when requested. - Triage and process referrals. - Upload comprehensive assessments. - To make referrals to other services and agencies as appropriate. - To attend partnership and service delivery meetings when required. - To bring to the attention of management any areas of specific concern relating to safeguarding of individuals or service delivery. - Develop a 6 Counties Recovery Resource directory. - Advocating on a service user’s behalf, for example by writing letters, making phone calls, or attending meetings. - Coordinate harm reduction interventions including Naloxone. - Deliver 1:1 and group interventions in line with psychologically informed practice and recovery framework. **Service Delivery and Development** - Coordinates linkage to Recovery Networks, building effective partnerships and working relationships with community groups, 3rd sector organisations and statutory services to have a clear understanding of local community strengths, resources, connections, identifying gaps and opportunities. - Coordinated support with the individuals referred identify their unique requirements and then supports them to access the community services or activities that they feel are right for them and seeks to provide this information on first contact with the individual using a "right first time" approach. - Works within the multi-disciplinary teams in community and prison settings, providing support and guidance for local teams on what services are available locally which may assist people in resettling more appropriately. - Helps identify areas of unmet need or gaps in service provision; evidence which is then used to help inform the planning of public sector and Third Sector services. - To support and develop volunteering and student placement opportunities across the service - specific focus on developing a peer-to-peer Community Connector scheme. - To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community. - Must demonstrate empathy towards Adferiad Recovery’s beneficiaries. - To carry out all other tasks commensurate with the role. - To complete comprehensive assessment and social identity mapping includin
