**Job Role**: Early Intervention Worker (Female Only)

**Salary**:£22,440 - £23,812 per annum

**Hours**:37.5 hours per week

**Contract**:Permanent

**Additional Information**:

- Full drivers license and access to car for business purposes
- On-call (1 in every 4 weeks), £100/week when on-call
- Participation in a Saturday rota, working approximately one Saturday in every four

**The Role**:
As an Early Intervention Worker you will support individuals experiencing rough sleeping to move into off the streets placements (short term assessment beds). You will work with this group both on the street and support them in the short term off the streets placement beds, including several emergency sleeping pods. As part of this Early Intervention role you will deliver high quality, person-centred assertive outreach sessions, working as part of a team, making contact with new, repeat and long-term people sleeping rough across the Exeter Council area.

Note: This will include working weekends and unsocial hours and being able to walk a number of miles in all weather conditions

**Key Responsibilities**:

- To undertake assertive outreach and hot spot visits across the locality area including during unsocial hours and weekends, maintaining a regular street presence
- Handle referrals and support individuals accessing emergency sleeping
- Carry out needs assessments, support plans and risk assessments with identified people who are sleeping rough to secure access to services that are suitable for their immediate needs (such as off the street placements), and make appropriate referrals in partnership with them to establish a sustainable life away from the streets as quickly as possible
- Empower a caseload of clients, to identify their strengths and goals, and develop their skills, confidence and well-being, so they can successfully maintain their accommodation and prepare for a positive move-on; avoiding a return to homelessness.

**Qualifications / Requirements**:

- Experience working with adults with complex needs and challenging behaviours
- Communication, building effective relationships with clients and dealing with multiple agencies
