Adults Hospital Service - Post Discharge Team
We are looking for individuals who are enthusiastic about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference and enabling them to increase their independence and have fulfilling lives, free from harm and abuse.
As a Social Care Facilitator in the Adults Hospital Service and Post Discharge Team, you will undertake assessments for people from short term services who have left hospital and are moving into long term services in the community. This is undertaken within a short time frame of up to four weeks.
You would be joining Solihull at an exciting time as the Council changes the way in which it delivers its services. We work in a more agile way where you will benefit from manageable caseloads, an inclusive and supportive team culture and great scope for professional development. You’ll work closely with Social Work Colleagues and benefit from high quality close supervision.
We are looking for someone who has:
- A Level 3 NVQ qualification in Health and Social Care (or equivalent)
- Maths and English GSCE Grades A*- C (4-9) or equivalent, or the ability to demonstrate an equivalent level of numeracy or literacy
- A flexible and innovative approach with an awareness of up-to-date legislation in the social care arena
- Experience of working within a social care environment
- Experience of delivering person-centred working
- Experience of assessment, the development and planning of care and of risk management
- Effective communication skills and experience of collaborative working able to build effective working relationships
- Sound knowledge of safeguarding principles and relevant legislation
