As a Quality Care Team Leader in the field of domiciliary care, you will play a crucial role in providing high-quality care and support to individuals within their own homes. Your responsibilities will encompass overseeing the care team, managing care plans, and conducting risk assessments to ensure the well-being and safety of our clients. This role requires excellent leadership skills, a compassionate approach, and a commitment to maintaining the highest standards of care.

Key Responsibilities:
Leadership and Team Management:

- Provide effective leadership to the care team, including assigning tasks, supervising activities, and ensuring adherence to company policies and procedures.
- Conduct regular team meetings, training sessions, and performance evaluations to foster a supportive and collaborative work environment.
- Motivate and empower team members to deliver person-centered care that promotes independence and dignity.

Care Planning:

- Assess the needs of clients and develop comprehensive care plans in collaboration with the individual, their family, and other healthcare professionals.
- Regularly review and update care plans to ensure they reflect the changing needs and preferences of clients.
- Ensure care plans are implemented consistently and that all care team members are aware of their roles and responsibilities.

Risk Assessments:

- Conduct thorough risk assessments for each client, considering their specific care requirements, health conditions, and environment.
- Identify potential risks and implement appropriate measures to minimize hazards and ensure a safe living environment.
- Regularly review and update risk assessments to address any changes or emerging risks.

Care Coordination:

- Collaborate with other healthcare professionals, such as nurses, therapists, and social workers, to coordinate and integrate care services effectively.
- Communicate with clients, their families, and external stakeholders to ensure a coordinated approach to care provision.

Documentation and Reporting:

- Maintain accurate and up-to-date records of client care, including care plans, risk assessments, progress notes, and incident reports.
- Prepare regular reports on the status of care provision, highlighting any concerns or areas for improvement.

Qualifications and Skills:

- Proven experience in a domiciliary care setting, with a focus on care planning and risk assessments.
- Leadership abilities with excellent interpersonal and communication skills.
- Strong organizational and time management skills to prioritize tasks and manage a team effectively.
- A compassionate and patient approach when dealing with individuals requiring care.
- Sound knowledge of relevant legislation, regulations, and best practices in domiciliary care.
- Ability to adapt to changing situations and make informed decisions.
- Proficient in documentation and record-keeping.
- Relevant qualifications in health and social care are desirable.

**Salary**: £25,000.00 per year
