What will you be doing?
You will be one of 4 Adult Support Coordinators, working along side Social Workers, Senior Social Workers and closely with LDP health colleagues.

You will hold a caseload of young people between 16 and 25 and will need to complete adult social care functions such as assessments, support plans, reviews, MCAs. You will also need to cover the team’s duty function a couple of times per month on a rota basis. You will work closely with education, health, social care providers, children's and adults' social care teams.
