**Wellbeing Support Worker - Mental Health**

A community-based organization that provides help and mental health support services for children, young people and adults in Stoke on Trent, across Staffordshire and other areas in the UK is now looking for a mental health service provider to join their team.

**Job Summary**:
Working within Midlands Partnership Foundation NHS Trust Integrated Mental Health Teams (IMHTs), providing input regarding the assessment and treatment of those experiencing severe mental illness, including assisting individuals to develop a Wellness Action Plan and providing peer and one-to-one support, psycho-educational programmes, recovery-focused activities and linkage to universal services and community assets.

**Key Responsibilities**:

- Assessment of new referrals from the IMHTs;
- Establish, deliver and maintain weekly peer-support groups, rolling psycho-educational programmes and recovery-focused activities (both face-to-face and digitally);
- Work with service users to develop a Wellness Action Plan to support their recovery and social inclusion;
- Develop relationships within MPFT and other Community Mental Health Fund commissioned services, e.g. Lifestyle, Housing, Finance etc;
- Support service-users to access relevant universal services and community assets;
- Ensure service users are actively involved in all areas of service;
- Ensure that monitoring systems are correctly implemented to provide consistent and quality data;
- Ensure that equal opportunities are fully implemented in all aspects of FFSS delivery;
- Follow all relevant policies and procedures as set out by MPFT and Changes: Health & Wellbeing;
- To undertake such other duties commensurate with this post as may be agreed between the post holder and Changes Board of Directors
- This is not an exhaustive list of duties and responsibilities and the post holder will be required to undertake other duties to meet the overall aims of the service delivery. The job description will be reviewed annually in light of changing service provision/requirements and any such changes will be discussed with the post holder. _

**Qualifications**:
**Essential**:

- One-year experience working/volunteering within a recovery/mental health service
- Basic understanding of the different types of mental illness
- Excellent presentation and facilitation skills
- Strong communication skills, both written and verbal
- Able to relate to a wide range of people from differing backgrounds, cultures and professions.
- Able to communicate clearly with a variety of people and to be impartial and non-judgmental
- Competent IT skills, a creative and dynamic problem-solver
- Ability to establish and maintain effective relationships both internally and externally
- Clean driving license and access to own vehicle
- Demonstrates exemplary confidentiality principles with a sound sense of personal integrity and ethics
- Proactive, motivated and open to change
- Able to plan and manage own time effectively
- Reflective professional with a commitment to personal learning, knowledge and growth
- A commitment to the principles of recovery and mental health.
- The ability to offer flexibility in order to meet the needs of the FFSS and service users

**Desirable**:

- A relevant degree or diploma
- First or second-hand experience of mental illness
- Involvement in/providing peer or one-to-one support
- Delivering training/psycho-educational workshops
- Providing/coordinating recovery-focused activities
- Knowledge of local services and community-assets
- Barriers faced by those with SMI in relation to education/training/employment/housing; employment etc
