Mental Health Recovery Worker

An exciting opportunity has arisen for a Community Recovery Worker to join the new and developing Home Engagement and Recovery Team (HEART).
HEART aims to provide bespoke, intensive, and individualised interventions and care to service users living with complex, enduring mental health problems to enable them to live fulfilling lives in the community.
The post holder will promote the independence and well-being of service users; assist social inclusion and enable the widest possible expression of each individual's chosen lifestyle.
As a recovery worker you will be working intensively with service users, enabling them to live their lives and achieve their own goals with support from a multi-disciplinary team of nurses, occupational therapists, social workers, psychologists, and medical staff.
The post holder will support individuals in their recovery and achievement of goals through using a variety of interventions to promote independence, functioning and social inclusion.
Interventions may involve activities of daily living, vocational and educational aspirations, development of family and social relationships, attending appointments, accessing meaningful activities, housing related support as examples.
The post holder will support services users in a variety of settings including their own homes, other accommodation providers and especially in the community.
The post holder will provide support to an allocated group of service users and promote their on-going rehabilitation and recovery.
They will assist the Care Coordinators and the wider multi-disciplinary team to assess, plan, implement and evaluate individual care plans.
The successful applicant will work under the supervision of the lead practitioner, focusing on the direct needs of service users working across boundaries of care and organisations.
The post holder will need to provide flexible working pattern that meets the needs of the service.
