Additional winter funding has provided the opportunity for Northwest Anglia Foundation (acute) Trust (NWAFT) to commission beds in a local care home to support patients temporarily who are medically optimised for discharge, but their discharge has been delayed. Cambridge and
Peterborough NHS Foundation Trust Intermediate Care Service are supporting NWAFT to provide rehabilitation for patients in these winter beds pending discharge home.
The postholder will undertake delegated duties from a registered clinician, they will have excellent communication skills, working closely with NWAFT colleagues and the care home to ensure continuity of rehabilitation and optimisation of patients’ function in preparation for discharge.
The postholder will ideally have experience of acute and community settings and will be able to manage a caseload of patients confidently within the interim care home setting.

- To work autonomously under the direction of a registered practitioner within the Intermediate Care Team in collaboration with care home and acute and community colleagues
- To be responsible for assessing, interpreting, planning and implementing therapy interventions for specific groups of patients
- To provide advice to patients and carers
- To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
- To plan and prioritise own workload and to provide a high standard of care.
- To work in the care home setting.
