**The post holder will**: Operate as part of a multidisciplinary team providing Crisis Intervention and Intensive Home Treatment to CYP countywide. Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to CYP. Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies, families and children and young people.
Actively and creatively engage CYP and carers encouraging them to make appropriate use of the mental health, social care and voluntary services available as part of a process of recovery and crisis management. Undertake full psychosocial assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure. Undertake a supervisory role to junior staff in line with trust policy and procedure.
Undertake acute risk assessment and safety planning for extremely vulnerable client groups during times of crisis. Deliver High quality, evidence based care to a defined team caseload Proactively support the prevention of YP requiring long-term mental health hospital beds, also encouraging and engaging in early discharge from same, should the need had been identified.
