Specialist Community Forensic Practitioner

We are seeking to recruit a motivated, enthusiastic, and experienced RMN/RNLD into the Specialist Community Forensic Team (SCFT) to take on the role of care co-ordinator.
The SCFT is a developing service supporting people who are identified as appropriate for step down from secure inpatient settings.
The post holder will be based in North Lincolnshire however the role does require travel out of county due to location of the patient group in in-patient settings.
The post holder will work closely and assertively with secure inpatient services to identify service users who are able to transition into the community and co-produce plans to facilitate this transition.
