North Lincolnshire Home first Community Reablement service provides time limited intervention to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to ensure that a person can remain at home and feel confident and is safe to live as independent as possible.

The successful applicant will have experience and be confident talking to a wide range of Health, Social Care professionals, external and internal and numerous universal organisations.

As a Reablement Worker the successful applicant will provide support to individuals following a change of circumstance or illness allowing a return to independence based on reablement principles. This involves following an agreed person centred plan and working in conjunction with the individual, universal services/ organisations, Health and Social Care professionals as necessary. Priority being to enable the person to continue to the task for themselves. Effective communication skills and the ability to provide support to complete complex forms or signpost to appropriate services is required.

Post holder will need to have experience of using digital recording systems and a number of digital technology systems to connect creatively with others.

The post holder is required to transport themselves around North Lincolnshire. Visits can range from South Killingholme to the Isle of Axholme, and a number of rural and isolated locations, there for transporting yourself in a timely manner is essential as several visits a day may be necessary and at short notice. Direct work include supporting people to develop life and home organisational skills, support with budgeting and managing the day to day aspects of their lives, appropriate housing as well as maximising opportunities for social inclusion, emotional wellbeing and independence.

The role within Home First offers a fantastic work life balance through a 23hr a week contract as a Reablement Worker.
