**Norfolk Children and Young People’s Services**: Provided in partnership by Cambridgeshire Community Services NHS Trust and Cambridgeshire and Peterborough NHS Foundation Trust.
Cambridgeshire Community Services provides the 0-19 Healthy Child Service (HCS). The post holder will work as part of skill mix team to promote the health and wellbeing of children, young people and families using an early help and partnership approach at every opportunity and contribute to improving health outcomes. The post holder will also be working under the supervision of a wider skill mix to support and deliver the Healthy Child Programme.
We have vacancies in our North and Broadland team based in North Walsham and our City team based in Norwich working Monday to Friday 09:00 to 17:00.
To support the integrated 0-19 Healthy Child Service (HCS) in providing an innovative and high quality service to improve Health Outcomes for children, young people and their families.
To assist the Clinical Team in the management and delivery of care to clients. Duties may include:
Contacts with families will be delivered in a variety of ways such as families homes and venue based development reviews at a variety of locations across the locality.
Building on existing assessment of families’ needs and deliver evidenced based health advice without direct supervision.
Receiving referrals, triaging and using appropriate sign posting.
Working jointly with Health Visitor for Specialist families. This may include communicating complex information with other agencies such as social care staff regarding work HCS are delivering to the family.
Support to clients with more complex needs.
Completing Duty - reviewing waiting lists and booking mandated development reviews and running reports within system one.
Working jointly with outside agencies such as the Early Childhood and Family Support service, Nursery settings and Speech and Language.
To support the assessment and review of clinical needs.
To work without direct supervision to support clients as part of the Universal and Targeted aspects of the service provision.
To liaise with colleagues within the clinical team to assist in the co-ordination and development of the service.
To develop specialist interest areas in order to enrich the knowledge and skills set within the Norfolk Children & Young People’s Health Service.

Maintain continued development and learning to ensure that practice remains current, evidenced and of high quality for the children, young people, and families we support.
To assist in the prioritisation of referrals and booking appointments.
To engage in team and locality meetings.
To offer an inclusive service that is accessible to all recognising individuals’ equality, diversity and rights.
Assist in the delivery and planning of programmes/activities of care that may be with individuals or to groups such as parents/carers.
To support the wider Healthy Child team in providing mandated universal developmental assessments in settings that meet the needs of the service user.
To provide targeted packages of care under the supervision of a registered case holder. Use goal setting to support the service user to identify their own needs and empower them to make sustainable healthy choices.
To write clinical records in a concise, accurate and timely manner in line with Cambridgeshire Community Services record keeping policy.
