Although you will be working independently in the community you will remain very much part of our wider nursing team, enjoying regular contact with managers and colleagues.

**Requirements**:

- Access to own transport
- Full clean UK driving license
- 2 or more years post qualified experience in a relevant field.

**Pay Rates: £19.00 to £25.00 p/h plus mileage allowance**

**As a Clinical Homecare Nurse, you will**:

- Be accountable for the competent and confident delivery of high-quality clinical care to patients across nursing services.
- Share careful assessment, planning, implementation and evaluation of care needs and requirement
- Assist referring centres, hospitals, and consultants in planning a smooth transition of patient care from secondary care to the homecare setting.
- Participate in clinical audit to drive service improvements as appropriate.
- Accountable for reporting all activity, risks, and issues in line with Wren Healthcare policies and procedures.
- Escalate to line manager or appropriate referring clinician any changes in any patient’s health and wellbeing or information that requires reporting.
- Assist with ongoing education and training of patients and families.
- Promote open and trusting relationships.
- Communicate complex and sensitive information effectively to patients, carers and other professionals.
- Participate in surveys, audit, and implementation of new services.
- Maintain confidentiality in line with all current legislation.
