Are you an Assistant Practitioner with a passion for frailty? We have an exciting opportunity for you to join a progressive, patient centred community frailty service as an practitioner.
In this role you will actively support the frailty pathways within an evolving integrated service to deliver evidence based frailty care and admission avoidance pathways across the local health economy. You will be required to assess and implement low level equipment to keep our frail safe in a home environment, you will be supported by our multi
- disciplinary team
You will respond to referrals in the community for patients with long term conditions that have rehabilitation needs. To assess and treat patients/clients individually and as part of multi-disciplinary team in their own home. To use knowledge to assist in the development of links with other community services.
They will be able to manage their own workload and work unsupervised supported by appropriate and regular clinical supervision
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
