**Salary: £21,840 per annum pro-rata for roles under 35 hours**
**Driving Requirement**:A full driving license and access to a vehicle is preferred but not mandatory**

Our new discharge to assess service in **Dunfermline** enables people, following a stay in hospital, to be supported and assessed in their own home to determine the type and frequency of any care and support they may need to live as independently as possible at home.

At the British Red Cross, we recognise the importance of taking a holistic approach to the assessment of a person’s needs following a stay in hospital. Our discharge to assess service carries out these assessments in the home. Our goal is to ensure people get the right care, in the right place at the right time. By providing support and facilitating access to resources, we empower people to regain their independence and confidence, all while maintaining their dignity.

This is a **Regulated Service** and you will play a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs and conducting assessments to determine ongoing care needs.

**Wondering about a day in the life of a Support Worker?**
- Conduct person centred assessments in the individuals home following a stay in hospital. The assessments ensure that people will receive the right care, in the right place at the right time.
- Work closely with people, their families, and other professionals to create care plans tailored to their needs and goals.
- Assisting and enabling individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, personal care and mobility support.
- Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.

**To be a successful Support Worker, you'll need,**
- Previous experience in a healthcare or social care setting is preferred but not essential.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
- Excellent communication and people skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving license and access to a vehicle is preferred but not mandatory.
- Registration with Scottish Social Services Council is required within 3 months of commencing in this role.
