**Support Worker - Discharge to Assess**
**Location**:Based within the community in Fife**

**Salary: £22,932 per annum, based on FTE of 35 hours (£12.60 per hour)**

**Hours: 35, 21 and 14 per week available**
**Contract: Permanent**

Join our dynamic team as a **Support Worker** You'll be at the forefront of our innovative approach, conducting assessments in individuals' homes with a person-centred focus on their needs, preferences, and goals. **What will a day in the life of a Support worker involve?**
- Work closely with people, their families and other professionals to create care plans tailored to their needs and goals.
- Assist and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
- Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.

**To be a successful Support Worker, what will you need?**
- Previous experience in a similar care, preferably within a healthcare or social care setting but not essential full training.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
