**Summary of Job Purpose**
To provide line management and leadership to a team of Senior Carers and Carers to ensure the delivery of high standards of care to Residents in accordance with current evidence based professional practice and within Porthaven policies, Care Quality Commission standards and regulations, and current legislation.
**Please note shifts will be 7-7 Days or Nights.**
**Key Duties and Responsibilities**
**Care Standards and Quality Assurance**
- To manage the floor or designated area throughout the span of a shift, using the handover and allocation processes
- To lead and co-ordinate the team to ensure optimum person centred care is delivered to Residents at all times and that Carers and Senior Carers work effectively within the parameters of their own job descriptions
- To implement the requirements of the Infection Prevention and Control Policy (CPol-54v23 Infection Prevention and Control - COVID-19 Revision) at all times
- To implement the requirements of safeguarding Residents from abuse and negligence
- Ensure the implementation of Pre-admission, Admission and Care Planning Policies
- Operate the Resident of the Day programme, ensuring that all required records are completed on the day
- To take on the role of Named Team Leader for a specific group of Residents and provide them with person-centred care planning, ensuring their wishes and preferences are respected and recorded at all stages during their care
- To attend stand-up meetings where required
- To ensure all clinical issues and concerns requiring input from the Nursing Care Manager or Deputy Manager are escalated in a timely and co-ordinated manner
- To identify and mitigate any potential risks to Residents, staff or visitors during a shift and ensure necessary documentation is completed in a timely manner
- To participate in the planning, implementation and management of care in conjunction with the Nurse or Nurse Associate for the shift, seeking and taking advice as required
- To participate in the review, recording and updating of care plans as required
- To be a competent role model and act as a knowledgeable resource for the team, always seeking advice from the Nurse or Nurse Associate as needed
- To ensure that all medicines are administered in line with the Porthaven Medicine Management Policy (CMed-01v2 Medicine Management)
- Ensure that medicine stock and clinical supplies are replenished in a timely manner and maintained as economically as possible
- Take responsibility for ensuring that the nutritional needs of each Resident are being maintained
- Work with the Leisure and Wellness Coordinators and ensure that the Residents leisure and well-being needs are being met at all times
- Demonstrate effective approaches to monitoring signs and symptoms of physical, mental, cognitive, behavioural and emotional distress, deterioration and improvement
- Provide support in meeting the needs of people in relation to rest, sleep, comfort and the maintenance of dignity
- Provide care and support with hygiene and the maintenance of skin integrity
- Provide support with nutrition and hydration
- Provide support with maintaining bladder and bowel health
- Provide support with mobility and safety
- Provide support with respiratory care
- Prevent and manage infections
- Meet the needs for care and support at the end of life
- Once trained, assessed and competent, undertake specific clinical procedures and tasks as identified below:
- Vital signs observations and evaluations of blood pressure, pulse, respiratory rate, temperature and oximetry
- Fluid and weight monitoring as well evaluation of urine output and bowels using Bristol Stool Chart
- Complete National Early Warning Score 2 (NEWS2), Delirium and Glasgow Coma Scale and stroke (FAST) assessments. Be able to recognise a health deterioration and escalate for medical assistance as agreed with GPs/ANPs or as guided by these assessments
- Perform urinalysis and collect blood specimens, urine, stool, sputum and swab specimens and ensure the specimen safe storage and transportation
- Where required, assist or perform male or female catheterisation and provide bladder wash out
- Be able to operate glucometer and measure ketones (in urine or blood) as well as evaluate the results obtained in this way
- Administer medication via subcutaneous and intramuscular injections, administer insulin and an emergency epipen safely
- Provide oxygen therapy and nebulizers, appreciating the difference between non-rebreathing mask and re-breathing mask, management of oxygen concentrators and if required provide support with positive airway pressure therapy (CPAP or BPAP machines)
- Perform aseptic technique and provide simple wound care and pressure ulcer management, including recording and evaluation of wound charts
- If required, provide stoma care management
- Oversee safe management of dysphasia, being able to identify and act when the management plan require
