An exciting opportunity has arisen to join the Poole Community Hub based at Alderney, as a senior clinician responding to patients in the community.
The post holder would be committed to supporting the management of patients with multiple, complex conditions, in need of urgent MDT intervention to maintain safety at home with the aim of reducing unnecessary hospital admissions and facilitate more rapid discharge from hospital.
The Poole Hub is a continually evolving team, working collaboratively with the wider community teams, forging partnerships with key services within the Community, Primary, Voluntary and Acute services to deliver holistic care to the patients.
The successful applicant will have excellent clinical skills; Physical examination skills, Non-Medical prescribing, a desire for knowledge, networking, inspiring others and a passion for putting the patient at the centre of their clinical and nursing care.
The post holder will work autonomously as an advanced
practitioner and have continuous responsibility for
proactively managing patients of highly vulnerable people
with long term conditions and complex needs in a
geographical area. The post holder will provide high level
clinical assessment, care planning, expert advice, extended
intervention and rigorous evaluation. To contribute to locally
agreed targets for reductions in emergency admissions
when appropriate.
The post holder will work across health, social and voluntary
services to initiate and develop care pathways to maximise
the quality of life for these individuals.
Facilitate learning and development of other practitioners
within the clinical area.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
