An exciting opportunity has arisen for a Discharge Officer to join the Acute Discharge Hub based at University Hospital Southampton. As a Discharge Officer, you will have responsibility for supporting the wards to optimise discharges and provide guidance and support for our more complex patients and relatives. ABOUT THE TEAM We are a small team of positive, hard-working, friendly, fun and resilient individuals who have a passion for supporting our more complex patients and each other. We have a "can do" attitude with a flexible approach to working. We always strive to balance work/life pressures which include team-building sessions, training and social events. The role will be full-time Monday to Friday, however, you will be required to work an occasional day some weekends due to the needs of the service. Our working hours are 08:00 to 16:00. Your Responsibilities What you’ll do As a Discharge Officer, you will be based on the wards liaising with ward staff, patients, relatives and external partner agencies. Each day is diverse and presents new and exciting tasks to undertake. We pride ourselves on working with and for our patients, advocating and supporting their wishes, whilst also assisting them to navigate discharge pathways. It is a busy and fast-paced role and we are therefore looking for someone who is dedicated to excellent patient care and has a good understanding of customer care skills. What we’re looking for As a Discharge Officer, you will need to have a keen interest in the discharge process and a passion to get it right for the patient. You will be expected to be a confident communicator with excellent organisational skills. You will be required to liaise with family members, patients and members of the Multi-Disciplinary Team on a regular basis, and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and often pressurised environment and be capable of using your own initiative.
