We are looking for a dedicated Healthcare Support Worker to assist us with patients in our domicilliary care business. The job post will mainly involve work in Telford and Wrekin, Shropshire.

The Senior Support Worker’s responsibilities include consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care.

Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness.

**Job Types**: Permanent, Fixed term contract

**Salary**: £20,480.80-£28,600.96 per year
