We're looking for enthusiastic individuals with great people skills and who love to take care of others. You'll be part of a driven team helping to support adults following a hospital stay to enable them to return to the comfort and security of their own home,or to prevent those in the community from going to hospital.
The CWTCH service is regulated by the CIW, and so you'll provide personal, practical and emotional support and care matching the individuals needs in line with CIW regulations in service user's own home. If needed, you'll signpost to other services to allowthe individual to live independently at home. It's a hugely fulfilling role, suiting those who love to care and support others to thrive.
The role will require you to register with Social Care Wales if you are not already a member.

The Independent Living & Crisis Response service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, socialcare, and emergency services.**This role could be for you if**

Also, you should be the following:
Educated to GCSE level (or equivalent by experience)
IT literate
Full driving licence (manual) and access to own vehicle
36 days holiday (including Bank Holidays)
