Health Care Support Worker Care Home Liaison

To listen and talk to patients and their carers and develop therapeuticrelationships establishing and maintaining professional boundaries. To carry out assigned tasks under the supervision of qualified staff including reviewing ABC charts and care home documentation to identify behaviours that are challenging to staff. To provide accurate holistic assessment of individual needs and support qualified staff in the participation of the care planning process. To provide training to care home staff to enhance their skills under the guidance and supervision of qualified staff.
To ensure effective communication, both verbal and written, reporting any change or unusual occurrence in an individuals behaviour or condition. To accurately report and feedback any concerns to qualified staff. To support care home staff in developing life story work with residents. To undertake and accurately document observations and care in line with Trust Policy and ensure electronic records are accurately maintained.
To understand the importance of, and maintain the need for confidentiality in all patient matters. To value diversity and promote equality of opportunity for all and to ensure individuals are treated fairly. To attend education and training sessions specially arranged for health care support workers as directed by manager. To act as a positive role model.
To attend / undertake statutory and mandatory training as required. To have an interest in self-development and be active in enhancing own personal knowledge including engaging in managerial supervision and IPDR. Adhere to appropriate Trust and local policies, procedures and guidelines. To contribute towards the development of the service.
Raise safeguarding concerns in line with trust policy and ensure immediate safety of service users. To undertake any other duties which may be determined within the general scope of the post.
