**JOB DESCRIPTION** **Post**: Advanced Practitioner **Reports to**: Clinical Manager and Practice Manager **Hours**: **Full or Part Time Hours to be considered** **Pay**: Band 8A Equivalent (£47,126 - £53,219 p/a) **Accountable to**: **Head of nursing - FCMS** **Base**: Office based **Hours of Duty** 8.30am - 6.30pm - flexible to meet the needs of the service **The Service** The practice is housed in purpose-built premises, alongside the local community hospital. Multi-Disciplinary working is at the heart of what we do, with both district nurse and ambulance teams on site here with the practice. The role offers the opportunity to work remotely providing primary care, as well as be involved in working closely with the local community hospital, which offers an interesting portfolio aspect to your future role. We are a well-established practice that cares for a population of 8,500 patients and we have been rated as overall "Outstanding" by the CQC. **Role** The post holder will act as an autonomous practitioner working independently within their professional boundaries to assess, diagnose and treat the conditions of patients You will provide expert advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. The post holder will be responsible for promoting and facilitating a healthy lifestyle and provide preventative health care and health education advice. The role will primarily consist of telephone triage - providing advice and clinical assessment when possible. You will also be required to action and request tests such as bloods, x rays, letters and ultrasound scans as able. The post holder will also contribute to administration and housekeeping duties within the locations. **Duties and Responsibilities** - Recognise and work within own competence and professional code of conduct as regulated by the appropriate professional body - Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures - Prioritise, organise and manage own workload in a manner that maintains and promotes quality - Deliver care according to local and national guidelines - Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making - Remotely Assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions - Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing the services - Interpret a range of diagnostic tests and routine clinical procedures e.g. cytology, venepuncture, blood pressure monitoring etc. - Work within all relevant policies and procedural guidelines e.g. infection control, chaperoning and risk management - Safeguarding responsibilities for adults and children and vulnerable groups, reporting concerns appropriately at the correct time and engaging with services as needed. **Clinical practice** - Autonomously assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and those with long term conditions in line with national and local policies and practice/ network guidelines - Diagnose and manage both acute and chronic conditions, integrating both drug - and non - drug-based treatment methods into a management plan - Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice - Work with patients in order to support concordance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions - Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills - Communicates highly sensitive condition related information to patients, relatives and carers - Initiates the process of diagnosis with patients suspected of have a chronic disease e.g. diabetes, COPD, asthma, IHD - referring to other clinical staff within the patient’s own GP Practice as appropriate - When required, ensure patients are referred to other members of the primary health care team e.g. podiatry, or secondary care as necessary - Clinically examine and assess patient’s needs from a psychological and physiological perspective and plan clinical care accordingly - Ensure the correct use of clinical templates, read codes and protocols - Inform the Clinical Manager / Service Coordinator of any untoward incidents e.g. falls, verbal aggression, criminal damage, safeguarding concerns - Contribute to infection control measures by adhering to infection control policies/procedures **Educational** - To follow safe working practices and t
