Clinical Lead

This opportunity is available to Nurses and Allied Health Professionals, on a secondment basis or 1 year Fixed Term contract (NHS pension/continious service is not affected)
As Clinical Lead, you will act as a key contact between the HUC service and Hertforshire Community Trust (HCT). Building and maintaining good relationships, to ensure the correct utilisation of Urgent Community response pathways.
You will provide remote clinical assessment, and care management to patients, offering advice or referral processes between HUC and community services, primary and secondary care. Working as part of a multidisciplinary team, you will form part of a wider Clinical workforce team, receiving patients as they present with undifferentiated and undiagnosed problems. Taking an active role in ensuring quality in line with organisational, national goals and strategy.
HUC provides accessible and high-quality Integrated Care services to nearly 3 million patients across a range of counties. When patients are ill HUC provides the NHS111 and Out of Hours GP service
