An exciting opportunity has arisen for a passionate individual with excellent link work/social prescribing experience to undertake a new role within Primary Care. The main focus will be on patients with low level mental health needs, helping them to access the support they need to live independent lives. Who do not have alternative forms of support and will not duplicate the work of existing provision locally. Referrals into the service will be identified by the practices. It is envisaged this will encompass people who have poor coping and problem-solving skills leading to a chaotic lifestyle and a reliance on primary care as a result of having no other options for support.
The essential feature of the role is to support adults with low level mental health needs by empowering them to take control of their health and wellbeing, focusing on ‘what matters to me’, and connecting people to community groups and statutory services for practical and emotional support. It will also involve creating action plans, reviewing these in a way that is most meaningful to the patient. Compliance with the recording requirements of the practices is essential to monitoring the effectiveness of the service.
Part of the role will involve patient triage to determine who in the practice would be best to support the patient. It is also envisaged that this role will develop to work alongside other third sector roles to support existing groups to be accessible and sustainable and help people to start new community groups.
You will be working within both bases of the Thornfield Medical Group: Molineux Street, Byker, Newcastle upon Tyne, NE6 1SG and Stoddart Street, Shieldfield, Newcastle upon Tyne, NE2 1AL. Your working hours will be Monday-Friday, between 8-6pm as directed by Thornfield Medical Group.

- Minimum of 2 years’ experience supporting people with mental health issues
- Are passionate about helping others realise their potential
- Are self-motivated and have good communication skills
- Are able to work to deliver required performance targets
