Job Summary:
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assisting in the coordination and management of care services for our clients. This role requires excellent organizational skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.

**Responsibilities**:

- Coordinate and schedule client appointments, ensuring timely access to necessary healthcare services
- Collaborate with healthcare providers and community resources to develop comprehensive care plans for clients
- Conduct assessments to evaluate client needs and identify appropriate interventions
- Monitor client progress and adjust care plans as needed
- Provide education and support to clients and their families regarding available resources and services
- Maintain accurate and up-to-date documentation of client interactions and care plans
- Assist in the coordination of referrals to specialists or other healthcare providers
- Collaborate with insurance companies to obtain authorization for services as needed

**Experience**:

- Previous experience in a healthcare or social services setting preferred
- Knowledge of medical terminology and healthcare systems
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Excellent communication skills, both verbal and written
- Strong organizational skills with the ability to prioritize tasks effectively
