The Care Coordinator will work closely with the clinical team and other primary care professionals within the Practice to identify and manage our registered patients, making sure that appropriate support for their health care and disease management is made available to them.
- Key responsibilities_:

- Proactively identify and work with a cohort of patients to support their personalised care requirements, using the available decision support aids.
- Bring together patient-identified care and support needs and explore available options with the patient to combine these into a single personalised care and support plan.
- Help patients to manage their needs, answering their queries and supporting them to make appointments or access specific GP services.
- Provide coordination and navigation for patients and their carers across health and care services, alongside working closely with social prescribing link workers and other primary care roles.
- Ensure that patients have good quality information to help them make choices about their care.
- Assist patients to access self-management education courses, peer support or interventions that support them in their health and wellbeing.
- Undertake work in line with PCN-directed priorities.
- Raise awareness of shared decision making and decision support tools and assist patients to be more prepared to have a shared decision-making conversation.

**Job Types**: Full-time, Permanent

**Salary**: £22,000.00-£24,000.00 per year
