We are looking to employ an Associate Practitioner to join our developing Flow Management Team in the Care Trust The post holder will work across both Inpatient sites of the Trust and be part of the Flow Management Team. They will work in a supportive environment that is committed to personal and professional development through regular clinical / managerial supervision and ongoing training. A flexible and solution focused approach is required as this post involves the need to work around the rapidly changing demands of our service users. **1. Job Purpose**: - The main purpose of this job is to support timely discharge and transfer of care of service users in the Care Trust and associate services / bed providers. This will be done by providing support for service users to find appropriate, suitable, and safe accommodation and benefits advice, alongside other factors that may influence delays in movement - Provide support as directed by the Flow Manager in the administration and completion of required documentation to assist the Bed Manage - Support Care Act Assessment completion via direct communication with Care Co-ordinators and the MDT - The post holder will be a point of contact for housing related referrals where the patient cannot return home or is homeless.
