Caremark Kirklees are looking for a dynamic, problem solving & organised Care Coordinator with great communication skills to join our busy and growing office team.

**Salary**: £23,000 per year + additional pay for being part of the on-call rota

**Hours**: Full-Time

Ideally you will have at least 2 years’ experience as a Care Coordinator, with NVQ/QCT level 3 in Care.

As a Care Coordinator you will coordinate our teams in the Holmfirth and surrounding areas. Principal responsibilities include:

- Answering all incoming telephone enquiries.
- Allocate Care Assistants to all customer visits and ensure all visits are always covered.
- Processing new customer referrals.
- Preparing appropriate documentation for Field Care Supervisors.
- Maintaining customer and Care Assistant office files and ensuring that they are always up to date.
- Ensuring all customer and Care Assistants documentation and electronic records are kept up to date at all times.
- Ensuring all other records are kept up to date.
- To work closely with the Field Care Supervisors.
- To ensure compliance with all office systems and procedures.
- Production of reports and management information as required.
- To share the on-call phone on a rota basis with the Field Care Supervisor and Senior Care Assistants.
