**Responsibilities**
- Plan and implement a variety of activities that meet the needs of the residents
- Coordinate with other staff members to ensure that activities are safe and appropriate
- Provide transportation for residents to and from activities
- Document activities and resident participation
- Maintain activity supplies and equipment
- Work with residents and families to develop individualized activity plans
- Promote resident independence and participation
- Attend staff meetings and trainings
- Other duties as assigned

**Qualifications**
- High school diploma or equivalent
- 1-2 years of experience in a healthcare setting
- Experience planning and implementing activities
- Strong organizational and time management skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Ability to work with a variety of people, including those with dementia and other disabilities
- Ability to work flexible hours, including evenings and weekends
