This is an exciting opportunity to be part of Wirral’s Community Integrated Response Team, where you be supporting people to remain independent in their own homes, using a reablement approach to help them reach their full potential.

Your daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation and social care and support, working with therapists and social workers to make sure people are achieving their recovery goals.

Care is provided to patients within their own homes.

An opportunity has arisen to work within a dynamic, innovative and friendly team. The Community Intergrated Response Team is going through a very exciting period of transformation, embedding the 2 hr Urgent Community Response, Virtual frailty Wards and Home first

There is a requirement that the lead nurse will provide strong clinical guidance and leadership.

The services provide care for acutely unwell patients in their own home (inclusive of residential and nursing homes), aiming to avoid unplanned admissions to hospital supporting earlier discharges from hospital

We welcome applicants from a range of backgrounds including emergency nurses, primary care and acute medicine as well as nurses from community backgrounds who have experience in working within Integrated Services.

The job role is required to work over a 7 day shift pattern, 8am - 20:00pm, Monday - Sunday.

About the Community Integrated Response Team (CIRT)

CIRT is an integrated team of clinical staff and professionals including; Nurses Physiotherapists, Occupational Therapists and Social Workers. The team have a strong focus on admission prevention and early supported discharged from ED and assessment areas. One of the fundamental principles of the Healthcare Assistant Role is to support our patients with their reablement and self care, encouraging them to reach their optimum level of independence.
