**Young Adults Transitional Workers** play an integral part of a multi-disciplinary team, within an all age, whole family substance misuse service across the London Borough of Newham. Our approach is centred around a resilience and strengths-based model of support that incorporates some core principles around prevention and earlier intervention, harm reduction and person-centred treatment.
An exciting opportunity has arisen for you to join a dynamic team committed to supporting young adults with drug and/or alcohol issues, and those engaging with the Y2A Hub (probation) other Young Person's Services in Newham, Greater London.
You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time you’ll be enjoying working with people from all backgrounds, making a difference to their lives. No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

**Salary**: £26,393.77 - £30,316.74 plus Inner London Weighting (£3,915.44)
**Responsibilities**:
**About the role**:
- Engage young adults and young people nearing adulthood who have substance use issues and provide them with appropriate information, advice, and range of interventions to support their health and wellbeing needs
- Build therapeutic relationships that are trauma informed and deliver a personalised package of care
- Complete comprehensive screening and assessments and develop individual care plans that incorporate integrated care and transition support that will enable progression in addressing the persons substance use and related outcomes
- Engaging with young adults and young people that is meaningful and impactful in a short space of time
- To advocate and empower young adults and young people with an emphasis on promoting the core principles around education and harm reduction
- Support young adults and young people with access to and engagement in education, training, and employment opportunities
- To be creative and innovative so that the post best meets the needs of the service user
**About you**:
- Experience of working with young people and knowledge of the issues they face
- Have an excellent understanding of drug and alcohol issues and experience of working within a related field
- Working knowledge of effects, risks, and harm reduction in relation to substances relevant to age range
- An accomplished communicator, both verbal and written with a high degree of personal IT competency
- Be able to accurately update and maintain records and to work to deadlines for the submission of information, e.g., reports
- Have excellent team working and interpersonal skills, maintaining a highly cooperative
