As part of our Integrated Care System, we are continuing to develop, transforming care across adult community services in our community hospitals and in people’s homes. We are looking for an innovative and enthusiastic individual focused on ensuring that quality improvement methodology is used to achieve continuous improvement and sustained change in service delivery. You will demonstrate an effective management, coaching and facilitation style, working restoratively within the compassionate leadership framework. You will proactively champion a culture where quality improvement is embedded in everyone’s daily life.
**The post holder will**:
- Be responsible for a designated portfolio of quality and patient safety within adult community services.
- Promote a culture of continuous improvement and patient safety.
- Monitor, interpret and present data to a high standard both verbally and in writing.
- Support regulatory compliance(Care Quality Commission).
- Have a key role in capturing and sharing learning, for example at the area Learning from Experience forum and across the organisation.
- Take a proactive lead role in understanding the experience of people who use our services and support co-design.
- Support/lead a range of clinical audits, collaborating closely with the Patient Outcomes and Clinical Effectiveness Lead co-producing action plans and monitoring outcomes for shared learning.
- Support/lead adult community services quality initiatives as required (for example CQUINS and the Quality account priorities) and attend monthly quality assurance meetings, preparing presentations and delivering data as required.
- Be aware of current research and how this can inform baselines for change work.
- Act as a role model, living the organisational values and aspirations.
We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Just over 532,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
