Below are the core responsibilities of the Safeguarding Care Coordinator role. This list is not exhaustive and will be reviewed on an annual basis or sooner if deemed necessary. There may be times when the post-holder is required to carry out tasks outside of the list below to assist with the needs of the Practice/ PCN: Administrative Support & Safety Management Serve as direct admin support for Safeguarding Leads within each network Practice Act as the first point of contact for patients, Practice staff, and external organisations for Safeguarding related queries and tasks To record patient interventions on relevant electronic database systems (e.g. S1),safeguarding coding and contribute to report generation, analysis and production Follow up on those children who have been identified as not having had their routine immunisations.

Discuss next steps with appropriate clinician (e.g. practice nurse) in-line with organisational safeguarding and other policies Follow up with children and vulnerable adults who do not attend/ are not brought to appointments Add alerts to parent records and those who have a child on a safeguarding plan, as well as reviewing and removing these if appropriate To ensure all electronic records are updated and complete within the agreed time-scales Prioritise and respond to strategy meeting invitations as advised by the organisations Safeguarding Lead Respond in a timely manner to queries and requests for patient information / summaries from social workers, and other services Respond to child protection meetings as advised by the organisations Safeguarding Lead, prepare appropriate documentation and send apologies as necessary Redact and update the clinical record as per organisational safeguarding protocols when a patient reaches 18 years of age Compile S47 reports Complete S17 and Child Protection conference information requests Liaise with local schools designated safeguarding lead using allocated inbox, and feed information to Practice Safeguarding Lead To manage patient-initiated calls for support/signposting, booking into named GP urgent care slots/telephone slots where necessary Conduct welfare calls to vulnerable patients and offer support To offer appropriate support, communication and guidance to patients and their families/carers May be given a caseload of identified patients and be required to ensure that their changing needs are addressed by considering local priorities, health inequalities and/or population health management risk stratification To support the population health project with liaising with identified cohorts of patients and other agencies To research appropriate websites & initiatives, downloading and circulating documents as requested to support patient care Policy, Processes & Audit Be responsible for running searches and daily updates of patients on electronic workflow management systems to identify patients with safeguarding concerns and keeping the safeguarding list / register up to date Monitor DNAs (Child not brought) for children regarding missed clinical appointments and refer to appropriate clinician for follow up (e.g., practice nurse, usual GP, Safeguarding Lead) in line with organisational safeguarding policies To be responsible for recording, reporting and producing evaluation reports which will include evaluation detailing effectiveness outcomes of new roles. Oversee the workflow ofsafeguardingreports and documents and ensure they are summarised, coded and circulated to appropriate clinicians in line with organisationalsafeguardingpolicies Maintain a register of children on Child Protection plans, Child in Need plans and those children deemed to be cause for concern Contribute to the integration of health and social care by maintaining up to date recording systems and providing information to any member of the relevant Practice team in order to ease processes and communication in agreement with data protection protocol Document and monitor aspects of patient co-ordination and service delivery, supporting data collection and audit using the patient administration system To use population health data to proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids Multidisciplinary Working Provide holistic support to the safeguarding leads and discuss patient related concerns, be able to follow appropriate safeguarding procedures to support the GP practices within the Primary Care Network Be responsible for arranging, facilitating and/ or attending Multidisciplinary Team Meetings, Proactively prepare any actions prior to the MDT ensuring all relevant clinicians are present To support clinicians and colleagues in the logging and makingsafeguarding-related referrals To work collaboratively with other teams and services to maintain an effective and efficient service To ensure regular and consistent communication with the referrer regarding patient progress and any
