**PCN3 Cancer Care Coordinator JOB DESCRIPTION**

**Job Title**:Primary Care Network 3 (PCN3) Cancer Coordinator

**Pay scale**:£25,000 per annum. Preference for full time, but flexible working will be considered. *Option to work Saturday and evenings as part of an Extended Access service*

**Accountable to**:PCN Business Manager

**Responsible for**: Supporting the PCN to drive improvements in Cancer outcomes.

**Location**:The post holder will be based at a PCN shared site, with some remote working. Travel to all the PCN practices will be required on an ad-hoc basis.

**1. Background**

An exciting opportunity has arisen for two enthusiastic and highly motivated
Cancer Care Coordinators to join the PCN3 workforce. The Cancer Care
Coordinator will be a key role in helping to drive improvements in cancer outcomes for patients through the delivery of early diagnosis of cancer in the
PCN, with a specific focus on improving the uptake of cancer screening by patients. The roles will also support our patients with new cancer diagnosis to navigate the health care system and ensure that their care is coordinated.

Our PCN consists of 8 GP practices serving a diverse population of over
70,000 patients in and around Barnet. We enjoy excellent collaborative working relationships. Our practices are all high-achieving and innovative training practices, and our ethos is to develop our PCN with a focus on high quality, holistic patient care. We are a strong PCN leadership team consisting of a
Clinical Director & Business Manager who are both aspirational and transformational. We require enthusiastic Cancer Care Coordinators who will complement the team through an enhanced passion and commitment to the

delivery of high-quality patient services and to the wider agenda of team working.

We have a rich diversity of PCN staff who aim to provide a personalised care approach for our patients, including Pharmacists, Nursing Associate, First
Contact Physiotherapists, Pharmacy Technicians, Care co-ordinators, Health and Wellbeing Coaches and Social Prescribers working alongside our GPs, nurses, and extended practice teams.

**2. Job Summary**
- The role will work closely with the PCN Business Manager, Clinical Director,

PCN Team leader, PCN administrative team, Clinical Leads, GP PracticeManagers, PCN Practice Managers.
- The role will have a priority focus=s on the Coordination & facilitation of

Cancer Care Services for the PCN
- To undertake Cancer reviews in line with approved frameworks for those patients with new diagnosis of cancer
- To support practices and PCN develop and review a robust electronic safety netting system.
- Support the patient process in the 2 weeks wait pathway ensuring that appropriate patient information literature has been provided and that the patient is in being seen under the 2-week pathway.
- To attend MDT where required to support coordination & facilitation of Cancer services
- To review and monitor cancer screening data for aligned practices.
- Help PCN & practices to understand why screening is being declined.
- To work with hard-to-reach groups to support and engage participation in screening.
- Actively follow up those patients who have failed to attend for screening, to achieve improvements in compliance.
- To maintain the necessary safety netting protocols for practices and PCN to evidence assurance
- To complete the required assurance reporting submissions under

DES/QOF/IIF
- Offer support & training to engage practice Care Coordinators in network of

‘Cancer Champions’ who can be supported by the PCN Cancer CareCoordinators
- The role is designed to support the PCN Business Manager to develop the

PCN and to offer a high-quality service to patients.
