**Social Worker, Older Persons Team, Paying up to £36per hour**

As an experienced Social Worker, you will be working as a Adult Social Worker based within Conwy so you must have strong statutory experience of providing excellent social care in a Adults team.

Duties include
- Undertake assessments, reviews, capacity, best interest meetings in the Community Adults team.
- Needs understanding of complex work and safeguarding.
- Generally based at home but expected to undertake visits
