We are looking for a Speech and Language Therapist to permanently join our team, working 2.5 days a week (18.75 hours) within Aspire Hub 2.

The post specialises in working with children and young people (CYP) aged 10-25 years who have complex communication and social, emotional and mental health (SEMH) needs, within the Aspire Looked After Children's Hub and Outreach Service.
- To manage a highly complex and specialist caseload independently, involving effective liaison and prioritising appropriately.
- To identify at a highly specialist level, the types of communication disorders found in vulnerable young people.
- To be flexible in working with a range of other professionals from other services such as health, social care and education.
- To offer appropriate communication advice and consultation their parent/carers and professionals working with them.
- To work closely with Aspire Hub and outreach staff to facilitate communication skills within the identified population.
- To plan and provide intervention options for this population, as appropriate, to include individual and group work (capacity enabling)
- To organise, plan and undertake training of staff and partner agencies to raise awareness of the types of communication difficulties and to provide strategies to facilitate their communication.
- To develop appropriate communication targets and resources (capacity enabling)
- To evaluate intervention outcomes (capacity enabling)
- To provide appropriate and timely highly specialist information to referral agencies, parents/carers, colleagues, and other professional staff involved in the management of the patient.
- To write highly specialist reports and communication profiles, including My Plan and EHCP reports, reflecting advice and knowledge.

Within Aspire Hub 2, the recipients will usually be aged between 11-19 years of age, or may be up to 25 years of age where the individual is a Care Leaver or has a special educational need and/or a disability.

The post-holder will be based within the Vulnerable Children and Young People’s Service, commissioned by Sheffield City Council. They will provide a speech and language therapy service to the CYP in contact with the Aspire looked after children’s hub and outreach service, as well as their parents, carers and practitioners working within the services.

The post involves the identification and support of SLCN at a highly specialist level for the type of communication disorders and co-morbid conditions found in this client group. This will involve assessment, attending meetings and direct intervention; including individual and group programmes involving CYP and their staff.

The post holder will work closely with staff and other professionals to facilitate communication skills within this client group. As part of this work the post holder will organise and plan the training of the staff and staff in partner agencies to raise awareness of the types of communication difficulties the CYP may present with and strategies to support their communication and SEMH needs.

The post holder will provide information and audit reports when required to the professional lead and commissioning Steering Groups.
