Prolife Healthcare Services has achieved considerable growth over a number of years and now provides care and support each week to people who are vulnerable in the society. Due to considerable growth we are now expanding into Trafford area and we have potential to triple before the end of this year if a goal driven Care Coordinator comes on board. **The Role** Reporting to the Registered Manager promote a high-quality service which promotes independence and choice to our service users enabling them to reach the optimum level of independence whilst living in their own home. To manage and provide relevant support to the care staff team. **Responsibilities** To liaise with the Care Quality Commission in accordance with the requirements of Care Standards as directed by the Registered Manager. Monitor and Review the practices, processes and systems within the service. Assist the Registered Manager to ensure all service documentation meets the quality objectives of the Company. Participation in the out of office hours paid on call "back up" service as required on a rota basis. Manage the day to day operation of the Branch and to cover calls when there is staff sickness, urgent cancellations etc Monitor the quality of the Service as directed by quality monitoring systems. Ensure home and office files are completed and up to date, documenting any changes as necessary. Ensure new files are set up with all the relevant documentation prior to the commencement of a new service. Attend Service User reviews as required. Complete care rotas for staff and service users ensuring that all daily care packages are covered on a weekly basis. Ensure all Service Users receive a weekly rota with names and times when staff will be covering their calls. Notify Service Users of any changes to their weekly rota. Ensure all Health and Safety procedures are implemented, monitored and reviewed as appropriate. Recruitment and selection of domiciliary care staff, induction, trainings organisation. Training matrix. To liaise with the service user, their families, social workers, GPs, CPNs, District Nurses and other key people regarding any care packages, ensuring their awareness is raised to any change of circumstance and/or to the initial assessments/care packages. To ensure that all staff are trained to the standards required and competent to provide the quality care required by individual service users. Essential Requirements (Knowledge, Skills & Experience) Care-Coordinator experience in a care setting. Compliance with the "Health and Social Care Act" Eligible to work within the UK To comply with all Policies of the Organisation, as appropriate. - To provide written reports of all complaints, accidents, incidents and untoward events in accordance with written Policies. - To ensure that the day-to-day Care Services are operated in accordance with Health & Safety legislation and current statutory regulation guidance on best practice. - To conform to the Company Code of Professional Conduct in relation to the operation of the Care at Home Service. - To participate in Staff, Appraisals, supervisions, Spot checks, Team and Quality Management Review Meetings as directed by the Service Manager. - To engage with and positively undertake on call duties as required by the branch to ensure a 24/7 service is provided.
