MMP Consultancy are currently recruiting for a Support Coordinator on a permanent basis, based in North Somerset. The Support Coordinator will work with an allocated caseload of residents to provide high quality support through 1:1 and group sessions whilstworking closely with external partner agencies.
Key Responsibilities:

- Work closely with the Managers and Support Workers to co-ordinate day to day operational needs of the service and ensure the effective delivery of quality support services
- Be involved in the referral and assessment process for potential residents, to include booking and facilitating assessment interviews when directed, recording referral paperwork and the assessment on the clients software and notifying the applicant of theoutcome of the assessment
- Support and accompany residents to attend essential appointments and signpost residents to relevant services and community-based projects
- Actively engage clients in their support and developing and improving the services
- Liaise with external agencies
- Develop person centred Support Plans and Risk Management Plans based on the residents' identified support needs to develop the skills and resources to transform their lives and enable independent living
Key Knowledge & Experience:

- Be able to participate in the 24 hour shift rota system (sleep-ins) and on call rota
- Possess a full driving licence and access to a vehicle that can be used to travel between sites
- Enhanced DBS check
- Experience of or knowledge related to working with homeless people
- Proven capacity to deal successfully with difficult and complex situations
