We are looking for a highly motivated specialist Frailty Occupational Therapist or Physiotherapist to take a senior role and work alongside the lead therapist ACP in supporting the therapy staff within the Urgent Community Response Team. You will be working closely and cohesively with a senior clinical leadership team of advanced practitioners in the team.

The Senior Frailty Therapist will have a variable caseload of complex patients who require crisis intervention and will be supported by advanced clinicians with clinical expertise and oversight to enable the optimal care of our patients.

You will offer bespoke supervision to other therapy staff within the team both occupational therapists, physiotherapists, and student therapists.

We work very closely with our system partners and are actively involved in the Hampshire Together programme. We expect that this role will work flexibly across community, and hospital services in line with the evolving integrated care model strategy.

We are happy to discuss flexible working but would expect a working week to look like: 8-4 for 5x days a week, or 8-6 for 4x days a week, including 2 days supporting the falls and frailty car and/or the urgent response van, Monday to Friday. There may be a requirement to work at the weekend to support the team where required.
