We're looking for a Patient Care Coordinator to work for our client, one of the UK's largest and foremost law firms. **Please note**: This is a full-time permanent role. You would work from home or the office, or a combination of the two, as part of the company's Flexible by Choice policy. You will be a close point of contact for the client and their familiesacross Leeds / Sheffield, full and clean driving licence required.
**The Role**
As a Patient Care Coordinator you will play a key role as part of the Personal Injury and Medical Negligence Team, working with a number of experienced and dedicated solicitors recognised for their expertise in supporting those affected by serious injury andillness.
You'll be tasked with the following:

- Acting as a link between lawyers, public bodies, agencies, charities, NHS and DWP, ensuring effective communication and securing additional statutory / voluntary services where required, providing on-going advocacy, signposting or practical support to clientsand ensuring the client is maximising their statutory welfare entitlement
- Liaising with clients who have sustained a serious injury or suffer from a serious illness, to provide condition-specific education, managing their expectations and providing support through treatment and rehabilitation
- Delivering a smooth transition of new clients from key acute NHS services and liaise with medical, therapeutic and care staff regarding discharge from acute care, providing advice and practical support to clients in the community or about to be dischargedto the community in regard to services and support available
In addition, it's essential that you possess the following:

- Experience working in a clinical, health/social care or rehab setting
- Understanding of cross-condition issues clients may have experienced (brain, spinal cord injury, orthopaedic injury, birth injury, industrial disease, bereavement)
- Experience of working in multiple settings (e.g. acute, post-acute, community settings)
