Assistant Practitioner

An exciting opportunity has arisen for the Integrated Care Service (iCares) in Sandwell to recruit a Band 4 Assistant Practitioner. You will join iCares working within the South locality team, that provides a multi disciplinary rehab service to people in their own home and in community settings.
iCares is a forward thinking, friendly and dynamic multi-disciplinary service delivering high quality and compassionate care to the people of Sandwell. The service provides a supportive learning environment for people with a passion for patient care in the community wishing to grow and develop as community health practitioners.
The role involves commuting between locations throughout the post holder's working day. Car driving is essential for this post, the post holder requires a valid UK driving licence and access to a vehicle.
- To work as an independent practitioner, liaising with all disciplines to provide a generic holistic approach to clients.
- To participate in the assessment, treatment and monitoring of own caseload of clients referred to iCares requiring rehabilitation in their own homes.
- To manage a complex and varied caseload of clients to meet patient and service demand
- To provide interventions to clients, working collaboratively with others.
- To refer onto other agencies as appropriate

We are a friendly integrated team who support each other and provide care within community settings across Sandwell. Training and regular supervision are available and no one works in isolation.
As a trust we fully support flexible working and part time workers, which can be factored into your working pattern. Within iCares we operate a 7 day service 8am-8pm, we take it in turns to contribute to weekend working and 8pm finishes.
