Assistant Practitioner

Job Purpose To work alongside Nurses, Physiotherapists, Occupational therapists and Assistant Practitioners within the virtual ward. To provide assessment and treatment intervention to people transferred from secondary care with an acute respiratory illness to the virtual ward within the Nottingham City and Nottinghamshire Healthcare. Facilitating the early discharge from hospital and caring for people in their own homes. To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.
Key Responsibilities Clinical To undertake individual treatment and interventions with clients under the supervision of qualified staff. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role. To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user. To promote wellbeing, safe mobility, transfers and effective communication.
To provide instruction, advice and intervention to service users and carers in line with the multi-disciplinary team guidelines. To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence. To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines. To attend and contribute to team meetings and case discussions using effective communication.
To maintain comprehensive and accurate records in line with employers polices. To promote health and wellbeing with all citizens. To undertake a range of nursing competencies, as per the AP competency framework. To operate under the guidance of clinicians and only within the scope of AP practice.
Managerial and Leadership. To provide support to other Assistant Practitioners as appropriate. Education and Development To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines). To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy.
Also, to keep an active up to date CPD portfolio. Clinical Governance. To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties.
To actively promote equality and diversity within service delivery and peoples individual rights. The post holder will comply with current legislation, the organisations policies, procedures and core values.
